a.Medical history for children/adolescents
Q5,6 and 7 are missing as per CRF spec form and Q10 should not be allow future date
b.enrolment medical history for all caregivers
Q13 should not allow future dates
Tested with PID 355-5 and 212-8

1. Maternal Med Hx form yellow page when adding date the symptoms started. see PDF attached.
2. Infant Med Hx CRF: The new changes reflecting are the new ones for the mother. Refer to screenshot for child changes
