Description
Changes to be made:
1. Trigger a repeat TB screening in 2 weeks for participants with symptoms.
2. If symptoms still exists within 2weeks or new symptoms arise, trigger TB referral.
3. For referrals already triggered:
a. Add field for reasons no referral was made: 1. Declined referral, and 2. No-longer has symptoms.

2 week repeated screening not triggered in case of positive symptoms see PID 0354-8 AT 3000 VISIT
It would be helpful to discuss this with Ame. I might be missing something

Hi Ame; UAT done and all the changes above have been noticed except for item #3.
Please refer to the master caregiver and child CRFs attached by Melanie for details on which additional questions to include in the referral CRF.

UAT done, all changes per the spec form included except;
1. Add "persistent symptoms" as the reason for the referral(Q3)
2. Add "other" as an option for no referral(Q1)
3. Include comment section on the" TB REFERRAL OUTCOMES CRF" under question 4(diagnostic tests) -for both caregiver and Child
