Description
1. Add Not applicable to question 21.
2. Add validation requiring this response only applies when there is miscarriage or stillbirth reported at question 16.

Tested and items above resolved however please add the following validation;
1. When stillbirth is reported at Q16,Q21 should always be "Not Applicable"
