while doing data cleaning, we realised we cannot capture well the reasons for exclusion from study,for patients who were not eligible, for other reasons other than the protocol stated exclusion criteria. These reasons are, but not limited to wrong phone number, no contact details or contacts not going through. Kindly add the following Qs to help us clean this data.

On question enrolment_interest, add answer option "UNKNOWN". then cresate another question after enrolment_interest, "if unknown, give reason" Make this answer option mandatory if UNKNOWN was selected on enrolment interest
