Description
Please add the following to the list of reasons on the Maternal Off Study Form:
"w: unable to confirm GA by Ultrasound"
Please update logic so that if the reasons "a, b, c, d, e, g, i, k, m, o, q, s, w" are chosen, this does not count towards accrual or recruitment cap.
