Infant side(TB Screening CRF);
Tested with PID 0683-0-10

1. Q9 and 10 are missing on the EDC, see the spec form
2. Change Q12 on EDC, to read as ...has your child(this part is currently missing) been evaluated...
3. If No, to Q12, that should be end of CRF; for now it still picks Q14 as required
4. IF yes at Q23, Q25 cannot be Yes
5.If No, at Q23, Q25 should be required

Infant/Child TB referral CRF
1.Use TB referral guideline attached above to trigger the referral form; the referral criteria is different depending on each age group e.g child less than 12 yrs, child more than 12 years
1.Q4 on EDC, reasons for referral are wrong-refer to the spec form and correct
3.TB referral form appear as required at 3001 visit- kindly remove(see pid 0066-8-10)

child TB Referral outcomes CRF
1. This CRF has not been validated-see spec form and add the validations accordingly
2. Add none option at Q4 spec form
3. Q7,8,9 on spec form, don't forget to start them with... was your child...for now it seems like the maternal side was just duplicated to the infant side without edits.

Child TB screening
1. Q14 should not be required if NO at Q12
2. Avail the TB screening as required at 3001

Child Referral CRF
1. If other is selected at at Q4, Q5 should be required
2. Remove the TB screening as required at 3001, FYI; it should be only triggered if there is a positive screening

TB referral outcomes
1. Q6 kindly fix the inline table
2. Ensure that all validations are in place from Q18-22
