Description

Please add the two following details at the top of the Off-study form for each participant once the form is opened:

1) The date of consent on their FIRST consent form (original consent on FLOURISH)
2) The response to the question about contact for future research studies (Y/N), on the participants MOST recent consent form (likely 4.0 for most but for those in secondary aims it may still be version 1.0 or 2.0)

Issue tested and resolved;

Note to the team; The additional question about contact for future research studies, will only appear to be asked again on the off study form ONLY for those who previously stated interest in future studies, otherwise the question would not appear, however Ame said where the question did not appear,the response upon CSV export will show as *Not Applicable"
